Safety Recalls

The Cascade River Gear Team makes every effort to contact our customers when important recalls regarding gear purchased from us is reported.  We will continue to post recalls and safety information to this page when we are made aware of them.  Whether you purchased your gear from us or another dealer your safety is our number one concern so please feel free call us with any questions you may have regarding the information on this page, we are here to help.

2014 – Important Safety Recall: We are conducting a voluntary recall of NRS Chaos Side Cut Helmets in cooperation with the U.S. Consumer Product Safety Commission (CPSC). We have discovered that a limited number of helmets were manufactured with faulty rivets connecting the chin strap to the helmet shell. When placed under pressure, the rivets can fail, allowing the chin strap to disengage from the helmet. If you purchased an NRS Chaos Side Cut Helmet between the dates of April 21, 2014 and July 7, 2014, cease use immediately and Check your helmet for the following;

Faulty rivets can lead to the chin strap disengaging from the helmet, posing a risk of head injury to consumers. Users of helmets subject to this recall should immediately cease using the product.

Identification: All NRS Chaos Side Cut Helmets purchased between the dates of April 21, 2014 and July 7, 2014 are being recalled. The recalled helmets have a “Boa” dial-fit retention harness inside at the back of the helmet. “Boa” is printed on the dial. “Chaos” and PO#39225 are printed on a white sticker inside of the helmet. Helmets with an “Inspected” sticker inside the shell under the “Boa” dial are not included in this recall.

Owners of NRS Chaos Side Cut Helmets subject to this recall will receive a
replacement, refund, or credit toward the purchase of another helmet. Please return recalled helmets to the point of purchase.

2014 – Important Public Notice: We have discovered that a limited number of Zen Rescue Life Jackets were manufactured with an incorrectly threaded top side adjustment strap, which prevents a tight, secure fit. Please read this important notice to determine if your jacket needs to be returned. (May require Adobe Reader to read.) 2013 – Important Safety Recall: Product Recall Notice On May 24, 2013, the U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration (PHMSA) issued Emergency Order Number 2013-002 mandating a recall of all cylinders manufactured by Lite Cylinder Company, Inc. (Lite Cylinder) of Franklin, Tennessee, and marked as authorized under DOT-SP 14562 (and DOT-SP-13957 as authorized therein), DOT-SP 13105; any cylinder requalified under H706, and any cylinders manufactured under M5729. This recall order also applies to any person who is in possession of an affected packaging that is subject to this order. The emergency order is based on an investigation of Lite Cylinder’s operations and production history that revealed probable violations and unsafe conditions and practices that PHMSA determined present an imminent hazard to the public safety. In effect, this action immediately orders the removal from service of more than 55,000 two-piece fully wrapped fiber composite cylinders. These cylinders are commonly used as portable fuel tanks for liquefied petroleum gas (LPG).

The following cylinder sizes are involved in this recall: 10#, 20#, 25# and 33#

Please note that the directive states “Under no circumstances should a cylinder described in this emergency recall order be filled, refilled, or used for the transportation of hazardous materials.” The cylinder’s identifying markings may be found on the label affixed to the dome of the cylinder.
